Thousands of nonprofits and social care providers serve their communities. But for most people, navigating the system to get help has been difficult, time consuming, and frustrating. Too many Americans are suffering, and they don’t need to.
Aging with Dignity
Aging with Dignity introduced Five Wishes in Florida in 1997, and a year later, to the nation. Dubbed “the living will with a heart and soul,” Five Wishes today meets the legal requirements in 42 states and has helped literally millions of people plan for and receive the kind of care they want. Five Wishes is unique among all other advance directives and living wills because it is user-friendly and easy to complete. Five Wishes Online was introduced in 2011, allowing people to complete Five Wishes on screen and print out a personalized document immediately. The document is available in 26 languages and in Braille.
The five wishes form is available from your LPO physician or online at agingwithdignity.org. Make sure to give a completed copy of your five wishes to your physician to keep in their records in the event that you are unable to access your own
File of Life
The FILE OF LIFE is an easy, free way to ensure that medical personnel have a quick record of your medical history, medical conditions and medications in the event you are unable to provide it yourself.
The FILE OF LIFE is available from your LPO physician.
Patient Centered Medical Home
The PCMH model was developed to strengthen the doctor-patient relationship and improve health care quality by helping primary care doctors and their health care teams more effectively manage their patients’ care.
To accomplish this goal, PCMH practices use such features as e-prescribing, chronic disease management, 24-hour access to a clinical decision-maker and patient registries to track and monitor care. Health care teams also work closely with patients to help them set individualized health goals and encourage them to take a more active role in their own health management.
